Remote Therapeutic Monitoring (RTM)

Connecting in-clinic care with at-home therapy to improve adherence, outcomes, and value.

The short answer

Remote Therapeutic Monitoring (RTM) connects in-clinic care with at-home and virtual therapy to improve rehabilitation adherence, measure outcomes, and reduce unnecessary surgeries, opioids, and imaging. Dr. Gruner helped develop the RTM CPT codes approved by Medicare in 2022. Early, consistent therapy can lower downstream costs by up to 60% per episode.

Why adherence matters

Rehabilitation only works when patients actually do it — yet only about 12% of patients prescribed therapy attend, fewer than 30% complete their sessions, and fewer than 35% adhere to their home exercise programs. Barriers include access, time, cost, and compliance. RTM is designed to close that gap.

Clinical impact

  • Improves therapy adherence and functional outcomes
  • Reduces emergency department admissions
  • Reduces unnecessary MRIs and opioid prescriptions
  • Reduces exacerbations and avoidable surgeries
  • Early, adherent therapy can cut downstream costs by up to 60% per episode

The RTM CPT codes

  • 98975 — initial setup and patient education
  • 98977 — device supply with daily recordings (30 days)
  • 98980 — treatment management, first 20 minutes
  • 98981 — treatment management, each additional 20 minutes
